  • Complete an initial screening form. Your informant will also contribute information at this point, with your consent
  • We will offer you three dates
  • Session 1 will be a face to face appointment in clinic
  • Session 2 will be an online appointment
  • Session 3 will be an online feedback
  • Report sent out with recommendations
